Everyone has in his or her personality some of these features, but in the narcissistic character they are chronically present.  One feels like a stimulus to a response which excludes participation.  In all cases, the resultant experience is that the interpretation was useless or even destructive.  Criticism of any kind, before a good rapport is established (which can take a very long time), may result in the patient's abrupt termination of therapy. 56 Unfortunately, this feeding of a person's narcissism is a seductive approach that often keeps narcissistic characters in therapy for a long time, with little transformation.

But if this rage is expressed, the positive purpose of idealization can begin to unfold.  His behavior will generally have a compulsive quality.  Commonly, this builds up in the patient into a rage reaction.  The result can then be a successful projection and integration of the idealized transference, leading in turn to the constellation of a stable, spiritual Self in the analysand.  Is there any difference between the integration process and its effects when a basically introverted approach is taken, and the more extroverted approach of working with the idealized transference?

Analytically, the eruption of rage when a sufficient analytical rapport has not been established is totally destructive, but equally, the denial of rage can result in a stalemate in which little transformation occurs.  In this manner, the narcissistic character often treats people in the way he experiences being treated himself.  The sense of specialness is continually inflated as a defense against the feeling of being hated.  Recovering this painful and frightening fact in analysis is essential, otherwise negative emotions remain in a repressed, splitoff condition, and grandiosity continues in a negative, defensive way, undergoing no change and in fact getting worse with age.
